ERIC surveys for children

ERIC wants to find out if and how bedwetting, daytime wetting and/or soiling are a problem for you. We also want to know how ERIC can help you and to ask you to help ERIC. So we have got 3 short surveys which ask you some questions.

These surveys are for children aged between 6 and 12 years. For surveys for 13 to 20 year olds, click here.

Important!

The surveys ask for your name and address, but you don't have to tell us UNLESS you want to be entered into the prize draw or you want to stay in touch with ERIC. As long as you complete the surveys, you can just give your name and address to be entered into the prize draw and we will only use your name and adress for that purpose.

If you give us your name and address, we won't tell anyone who you are or where you live without getting your permission. The only time we can't keep your name and address confidential is if you tell us something that makes us think you might be in danger.

Make sure you ask your Mum, Dad or whoever looks after you if it's OK to give your name and address.

Prizes for 5 lucky children!

5 children will get a prize as a thank you for taking part. To be in with a chance to win a prize, you have to fill in and send back all 3 surveys and be willing to give your name and address, as we will pull names from hats. The completed surveys need to be at ERIC by the end of September 2008.

The children's prizes are a £50 voucher, 2 £20 vouchers and 2 £10 vouchers. You can choose to have a voucher from WH Smith, Woolworth or Zavvi. The first name pulled out will get the £50 voucher, the next 2 will get the £20 vouchers and the next 2 will get the £10 vouchers.
